Suggest Treatment For Fluctuating Sugar Levels

My mother get admitted nursing home on 14th on this month 2011 at hypoglycemia at unconscious stage, same thing happened on 26/10/2011 and she was admitted and along with Na+ fall & K fall . Now she is at home , but suger is uncontrolled Last three days result was 200, 233, 267, 233 (today morning ) , 321 ( today at 1PM) and 361 at 5 pm , As per doctor advice we applied glypride 2 at 9 am and 9 pm every day . Previously we applied insulin 6 unit at morning and 6 unit at night , after releasing hospital from 19th on this month ..., But we afraid of sugar falling , can you please help me , currently my mother is lying on bed and gets urine at bed can not sit in the bed.

Dear sir i can understand your concern.
I am very sorry about the condition of mom.
She will be alright soon.
In my opinion the timing of food and medication is very important in avoiding low sugars
In this kind of conditions a combination of ultra short acting insulins like novorapid and long acting insulins like levemir and lantus will work better than tablets like glimipride.
KIndly check sugars once in 4 hours prepare a chart,small dose of short acting insulin can be used if sugars are going above 250 mg/dl this method is called sliding scale.
I suggest you to take advise and necessary information to follow sliding scale method at home.
Most of my patients will in touch in watsapp or message for adjusting there insulin levels from home.They are able to manage very well from home.
Dont worry mom will recover soon.
