Hi,
Welcome to health care magic,

Lower abdominal pain, fever, urination with red stools could have many causes which are gastric ulcer or Helicobacter pylori infection or bleeding from large bowels.

You should undergo routine blood works, liver function tests, urinalysis and stool for blood, abdominal ultrasound and lower gastrointestinal endoscopy (colonoscopy) to detect the underlying cause.

Low WBC count has nothing to do with this fever or lower abdominal pain but should be further evaluated by above tests.

Treatment is mainly depend on the cause but you may take symptomatic treatment meanwhile.

Antacids like h2 blockers (ranitidine) or proton pump inhibitors (pantoprazole or rabeprazole) will help to reduce excess acid production in stomach.

One can also take digene gel or liquid or ulcero-protective agents like sucralfate or bismuth subsallicylate to reduce the burning sensation.

Apart from medications she should eat plain foods like boiled rice and green leafy vegetables without adding spices or oil. It will help to neutralize digestive acids and also they are easily digestible means there will be no need of extra stomach acid.avoid oily, spicy and fast food

Consult gastroenterologist for detailed medical history and clinical examination to diagnose the cause.

Thanks and Regards,
