Suggest Treatment For Insomnia Caused By Mental Disturbance

i am 50 year old man. i have a kinda crazy problem that have caused a chronic insomnia. i have one particular phrase that pops up in my head to irritate and bother my sleeping. it is crazy words chop with an ax this phrase invites a gruesome mental image and chills away my sleeping. That words have been stuck in my head for many years. what is it? ocd or ptsd. i started to take paxil. will it help with the problem? desperately need help.
