Suggest Treatment For Neck,back And Knee Pain

Hello,

I am a 30 year old male and am experiencing major neck pain (almost 16 years now), minor lower back pain, and left knee pain (which has recently turned into random complete failure of the joint while walking).
The neck pain started originally after a football accident where i was face masked and landed directly on the top of my head. After about 2 weeks of rubbing my neck I heard a loud pop and was able to turn it side to side and the pain was reduced to a minimum amount. All X-Rays of my neck and knee have returned that there is no problems and that both look healthy. I recently requested an MRI of my back as my doctor will not give me anything for the pain and keeps telling me there is nothing wrong. In getting the MRI of my lower back they found that at the L4-L5 level, mild concentric central canal narrowing. No significant neural foraminal stenosis.
There is also some minimal dialation of the central canal of the spinal cord within the conus medullaris.
L4-L5 disc level there is a mild diffuse bulge.no protrusions or extrusions present. There is some mild hypertrophy of the ligamentum flavum. There is some mild lateral recess stenosis but no displacement of the transiting nerve roots.
There is some very minimal impingement on the left neural foramen perineural fat.

Can anyone give me some feeback. I tried to include as much info as I could.
