Suggest Treatment For Pubococcygeus Muscle Spasm

how can I diagnose and treat pubococcygeus muscle spasm. I am a 68 year old man who is also on flomax for BPH. I have had a normal cystoscopy, pelvic CT, normal PSA (1.0) and normal exam by my Internist and Urologist. But, I continue to have nocturuia, urgency, hesitency and tightness and pressure in my perineum, rectum and tip of the penis.

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

The pubococcygeus muscle spasm can be best evaluated by resting tone measurement via pelvic floor muscle manometry (perineometry).

The management depends upon the intensity of the issue, which can be Botox, physiotherapy and other supportive measures.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
