Suggest Treatment For Severe Rectal Burning And Pain During Defecation

Hello, I have a serious problem and am looking for suggestions, I had a total colectomy, ileo-rectal anastomosis with diverting loop ileostomy in 8/2014. I subsequently had a leak and peritonitis, I had pelvic floor dysfunction (reason for ileostomy) and received bio-feedback treatment. In February I had the reversal surgery and had no complications, I feel great except I have begun to experience severe rectal burning and pain with defecation. It feels like I have to bear down even though I have just relieved my bowel. I did have a stricture at my anastomosis and had that dilated. This burning is unbearable when I am in the bathroom. I have used hemorrhoid cream, dimethicone barrier cream and am now using Desitin, the pain sometimes is intense and I avoid eating because I have to go to the bathroom every time I eat within 20 minutes if not less. Is there anything I have changed my diet, but nothing seems to relieve it, I am however aware of what makes it worse. If there is any remedies that any one is aware of that would help, I would greatly appreciate it.

Severe rectal burning and pain during defecation after surgery can be a sign of an anastomotic leak, inflammation or infection, stricture, or hemorrhoids. Since you have already been diagnosed with stricture and had it dilated, it is possible that the stricture has recurred, causing pain and a burning sensation. Another possibility is that you have developed hemorrhoids or anal fissures, which can be quite painful.

Treatment options for rectal burning and pain during defecation depend on the underlying cause. You may need to have your anastomosis re-evaluated by your surgeon or a gastroenterologist to determine if there is any recurrent stricture or inflammation that needs to be treated. If hemorrhoids or anal fissures are the cause, your doctor may recommend stool softeners, fiber supplements, or topical medications to reduce inflammation and pain.

In addition to medical treatments, some self-care measures may help relieve rectal burning and pain. These include:
- Taking warm baths or using a sitz bath after bowel movements
- Use cold compress or ice packs around the affected area
- Avoiding constipation by drinking plenty of fluids and eating a high-fiber diet
- Using over-the-counter pain relievers, such as acetaminophen or ibuprofen, as directed by your doctor
