Hello. Fournier s gangrene and stricture urethra are 2 different entities. Fournier s gangrene is the decay of the skin and surrounding tissue near the scrotum and nearby areas. Although tje exact cause is not known, it is said to start as a result of an infection the in urethra or testicles or as a result of any trauma or anal fissures or fistulas. It is most commonly seen in diabetics and elderly people. It is a life threatening condition as it is a severe infection. Since you had it I presume you have been treated adequately and are now stable and out of danger. You are right in suspecting stricture urethra as a possible cause for poor urinary stream. A common cause is trauma or infection like gonorrhea (that which occurred more than a year ago will cause symptoms now). Another very common cause that needs to be considered especially if you are elderly is prostate enlargement. It causes similar complaints like inability to void urine, poor stream, urgency and hesitancy to pass urine. Your surgeon can diagnose it by a rectal exam and which will be further confirmed by an ultrasound. Serum Prostate Specific Antigen levels will help rule out any tumor of the prostate (needs to be done before a rectal exam). Hope this helps. Regards, Dr Geeta
