General & Family Physician 's  Response
Hello,
Thank you for your query.

Yes, please take him to the ER.
I recommend :
- immobilize the arm (splint if possible) or keep his hurt forearm across his chest with his fingers near his opposite shoulder.
- ibuprofen dosed for his age and weight - one dose
-an x-ray to check for any fractures.
-apply ice pack (wrapped in a thin cloth) over the injured area
-avoid trying to check if it hurts by flexing or rotating hand and fingers.

If it a a simple fracture (without any displacement or multiple breaks), cast for a couple of months will suffice along with Pain killers.

If it is a muscle or soft tissue injury, ice compresses, elevation of the arms, pain killers & anti inflammatory drugs will do.

Sometimes if the damage cannot be fixed conservatively, he may need surgery (unlikely based on the description of his injury and symptoms)

Hope this helps.
Please let me know if there is anything else I can help you with.
Wish you good health and speedy recovery for your grandson
