Hi,
Thanks for posting your query.
Let me know since how many months your wife is taking medicines.
MDR TB is diagnosed only after 4 months of treatment with no improvement (as defined by WHO guielines).
Spread of TB from Lymph node is possible after contact of fresh wound with TB infected lymph nodes, infected hands contaminating edibles.
Spread by breathing is possible when simultaneous lung infection is also persisting.
In case of any doubt, you should go for AFB culture & sensitivity of pus/ tissue from infected lymph node.
In this way, status of affected medicines against infected TB may also be known.
Take care,

Hi,
Thanks for asking Health care magic.
As you have mentioned your spouse is having Extrapulmonary tuberculosis, that is TB outside the lungs.
The treatment regimen is SAME as that of the Pulmonary TB. And we SHOULD NOT brand a person as having MDR-TB (which is defined as resistance to isoniazid and rifampicin) unless they are resistant to the above said drugs with or without resistance to other first line drugs.
TB Bacillus spreads through the air. The droplet nuclei generated when a sputum positive pulmonary TB patients coughs, mixes in the air and are carried to a susceptible person in the vicinity or by air currents to longer distances.
Effective anti TB treatment reduces the infectivity by 90% that is, the communicability is reduced by 90%.
TB is not transmitted by touching, sex or eating in same utensils used by the patient. unless some discharge is coming from the lymph node site (which should be disposed as recommended by the Govt) no special precaution are needed .
MAKE SURE, your spouse completes the treatment regimen as recommended by the Government and complete the DOTS (Directly Observed Treatment Shortcourse). she should do well.
take care
regards
Dr.K.Muthukumar
