Welcome to Healthcare-Magic
Greetings of the day

Penicillin prophylaxis is only required in documented cases of Rheumatic fever/Rheumatic heart disease for preventing streptococcal infection . In your case if mitral regurgitation has been documented to be due to Rheumatic diseases, it should be taken for atleast 5 years since the last episode of Rheumatic fever .

I hope it helped

Take care
