Was Diagnosed Typhoid And Had Treatment But Fever Still Persists. Had Tests Done. Findings?

Hi Sir, Hello. I am Rajesh from India, diagnosed Typhoid in Feb 2013. Since then undergone various medications but fever (Low Grade) still persists. 2 days back, had blood investigations and major concerns were:
ESR- 85 SGOT- 47 SGPT - 53 S. Alkaline phosphatase - 245 Platelets Count - 3.67 MCH - 32.1 Triglicerides - 206 VLDL Cholestrol - 41 and Typhoid all +ve. please advise.

Hi Rajesh, Thanks for using HCM.

Even after treatment you are still positive for typhoid and with raised ESR and liver enzymes suggest you to restart the medications for management of typhoid. After consulting your doctor, you can take Ciprofloxacin 500mg twice daily after food for 14 days without missing any dose and other symptomatic treatment like paracetamol 650 mg Three times a day / SOS, reduce oil / fat intake, take more of carbohydrate food. Syp Liv 52 may be helpful.

If you had already used ciproflox for 14 days then this suggests resistant case and you may require azithromycin 500mg for 8 days with first dose of 1000mg. This will help you.

Hi,
Thank you for your query on Healthcare Magic.
The ESR is high, but it is a nonspecific test and other tests may be required to know the causes for its elevation.
ESR is elevated in many conditions like chronic infections, anemia, and other chronic inflammatory problems.
SGOT and SGPT are in normal limits.
Triglycerides are high.
What is the titer of Widal test?
Widal test to be interpreted with the duration of fever, other clinical symptoms and clinical findings.
Typhoid antibodies will persist for longer duration even if the typhoid fever is treated effectively with antibiotics.
Hence I advise you to consult the physician for interpretation of the reports and to advise you an appropriate management.

if the fever is repeating for many days you have to get a detailed lad investigations done- Tb work up- TB gold, Blood c/s, chest XRAY, usg abdomen, review with reports.
