What Are The Chances Of Transmission Of The HBV To The Newborn?

A 26years/F , G2P1 at 39wks+5days of pregnancy presented with pv leaking and abdominal pain and on pv examination the os was fully dilated. Her HBsAg done at the very time was reactive which was reactive in previous blood test also. What would be the further management and chance of transmission of the virus to the new born? I m a staff in one community hospital and want to know this things. Would you help me doctor?

Has she been tested for HBeAg also?
The risk of transmission hepatitis infection from infected mother to baby ranges from 5-90% in the perinatal period without immunoprophylaxis. Mothers with positive HBeAg result have a transmisssion rate of 70-90% and those with negative HBeAg result have a transmisssion rate of less than10%.

This risk of transmission in mothers with positive HBeAg is
reduced to 5-15% and in mothers with negative HBeAg it is reduced still further with immunoprophylaxis to the baby after delivery. Immuno prophylaxis includes giving Hepatitis B immunoglobulin and first dose of hepatitis B vaccine series within 12 hours of babys age. The second dose of vaccine should be given at 1-2 months age and third dose at 6 months of age.
The mother can breast feed her baby as long as immunoprophylaxis is being given.
It is extremely important that the staff in labour ward are made aware of mothers condition and all precautions should be taken during delivery to prevent transmission to the staff.
