The commonest causes I can think about are:
Diabetes in itself causes the skin to lose its integrity and thus easily gets succumbed to anything like urine and stool. Hence the first thing to do is to get the vitamin levels in blood checked and to get proper supplements under the advise of your Doctor, particularly for
Vitamin A and D.
The urine and stool have lots of bacteria which cause easy infection of the skin in contact, repeated every time it comes in contact and thus easily gotten
rash.
For this you have to use the skin protecting
ointments.
This will solve your problems.
Whenever needed you can go for urinary catheter and proper bowel care and habit to avoid much of the problems.
Also get the urine and stool tested and treated with antibiotics if there is any evidence of infection.
Also use probiotics as these help for normal skin.
