What Causes Abnormal Ear Discharge After Eardrum Repair?

My daughter had her eardrum repaired 4 days ago. She has been having a lot of thick discharge, just like prior to surgery. Today she found something like paper in the gunk. If her patch fell out could it be possible the Dr applied to much ointment in her ear canal leading to the failure? Should she have been given antibiotics post surgery?
