Hello,
it's hard to tell. Blood could be due to minor injury during the fall or could be a sign of pulmonary edema (taking into account his 'cardiovascular' history). Patients with pulmonary edema may experience frothy blood-tinged sputum. Cardiovascular patients can be very upset in the event of an infection (which is implied by his 'hot' temperature).
Since nobody witnessed his fall it's impossible to know what happened. Only autopsy could have shed light to the cause of his death.
Kind Regards!
