What Causes Chronic Joint Aches Along With Elevated Liver Enzyme Levels?

Suffer from incredible pain in hands, feet, other joints coming from both gout and neuropathy. My ALT and AST increased from 50 to 250 and the other from within the range to 90. Can t take Alapurinol (sp.?) and stopped Acetaminophen. Wake up at night with burning hands and feet. Daytime brings pain from my changing feet. It s a nightmare. My primary doesn t want me to take pain medication. I went to specialist who is focused on my back. My gout doc can t do anything until my liver gets better and she spoke of becoming a vegetarian. I am 74 years old. Had two bouts of lung cancer. Hip replacement inbetween years. Severe lung disease before and after and am on 02 24/7. I m a fighter who has been going to the YMCA swimming when I could with 02 bottle on my back. Right now I am needing to live. But not with all the pain. Anyway, Oncologist is tracking what s going on in my brain 4 MRI s in 4 months. This Tues I think I will hear horrible news. What do I do with all of this? This pain in my feet and hands is on me when I am at my most vulnerable. Am I looking for a miracle? Uh Huh! Yup!

General Surgeon 's  Response
Hi,

The more common causes of increases in AST and ALT can be due to:
(1) Non alcoholic fatty liver disease - this is one of the most common causes as present. It is due to build up of fat in the liver which can then go on to cause inflammation and damage to the liver if it is allowed to progress. Treated with diet changes, activity and medications
(2) Alcohol - persistent use of a lot of alcohol causes damage
(3) Side effect of medications eg statins for cholesterol. This does not happen to all persons who use the statins, only some
(4) Infection - hepatitis is another cause of abnormal liver enzymes
(5) Hemochromatosis- iron is deposited in the liver. Your doctor may suggest an ultrasound or CT of the abdomen to take a look at the liver.
Additional bloods, if not already done, may also be requested to determine the cause eg complete blood count, cholesterol levels, iron and the hepatitis viruses

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Iven Romic Rommstein, General Surgeon
