General & Family Physician 's  Response
Dear Patient,
Thanks for contacting HCM
Firstly i must congratulate you for the healthy life style you are living and also don't be discouraged by slow loss of weight rather you have reduced your weight by 18 pounds which is great. I generally don't recommend weight loss medications to my patients and encourage them to do regular exercise and have a balanced diet.
I would recommend you to go for brisk walk or jogging for at least 30 minute for at least 5 days in a week (by brisk i mean you should not be able to complete a sentence in a single breath while walking) and cardiovascular exercise 3 times in a week, but before going in for walk or cardiovascular exercise i would recommend stress ECHO.
You have said your BP is 140/100 despite taking Metopral 50 mg, so i would recommend addition of other class of drug like Enalapril or Ramipril (belonging to ACE inhibitor) or Amlodipine ( Calcium channel blocker). All of these are prescription drugs and your treating physician has to prescribe them.
Regarding chest pain, if you were my patient i would like to do Ecg, ECHO, lipid profile, fasting and postprandial blood sugar. Mean while you can take Capsule Omeprazole 40 mg half an hour before breakfast. Depending on the test results further line of therapy can be decided.
Hope this has helped you
Regards
