What Causes Extreme Hypertension, Memory Loss And Petechiae On The Arm?

i am working with a patient whom has been experiencing extreme hypertension, memory loss and today noted a very defined petechiae along his left arm from his wrist to his elbow. He is also on hemodialysis. What is the cause of the rash? Could the etiology be related to a brain disorder?

General & Family Physician 's  Response
Hi,

Based on the history you have shared, neurological signs can be due to hypertensive encephalopathy. With patients history uremic encephalopathy may be ruled out.

Since chronic kidney disease is a prothrombotic state so the possibility of ischemic stroke should also be ruled out.

Also, uremic coagulopathy should also be ruled out on platelets function tests and coagulation profile. Desmopressin may be helpful in that case.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
