What Causes Grand Mal Seizure Along With ADHD And Sleep Apnea?

My husband is 60, has ADHD, had a trauma bike accident (concussion-hit front of head) 14 years ago, recently diagnosed with severe sleep apnea with AHI 88, most of the night in rem stage. Had a grand mal seizure in the middle of the night a couple of days ago--not sure why. No seizures showed on the all night EEG. Head CT and MRI normal for his age group, blood tests normal, vitals normal. Any ideas on what caused the grand mal seizure?

Certainly, it is known that following any type of traumatic brain injury (TBI) convulsive seizures can occur. Most of the time they occur within the first several weeks to several months following the accident but occasionally it can be years. Often times, MRI studies looking at the correct area may show "scarring" or even evidence of residual bleeding. Otherwise, it could be a host of other things that would have to be ruled out by thorough investigation. Typically, we will do several EEG's under different conditions to try and capture the seizure type so they can be adequately treated.
