What Causes Hardness In Breast And Fever While On Hormone Replacement Therapy?

I am female, 63 years old and started homone replacement therapy at the age of 46. I have been using bioidentical hormone therapy for 13 to 15 years. I quit having a period at 46 (because of the homone therapy) and did not have one for years until recently. This month, I have had pretty severe cramps and bleeding three times. The first time was only a couple or three days and not much blood. The second time was three days and more blood and the third time was like the first time. I did not have the blood flow or the severe cramps like I did when I had periods. Also, I have one breast that is larger and hard with a fever. It is not like a lump. The whole breast is hard. What does this sound like?
