What Causes Headache And Bruise On Forehead After A Head Injury?

Was in a head on car collision at 50 impact. Next day it was pointed out to me I had a bump and nice size bruise on my right side on my forehead. This was last Aug 2013. This bruise is still there and it flares up at times its noticeable difference from one side to the next. I m getting really bad headachs to the point I had to put ice on my head to try to help the pain. Yes had a ct was told fine no bleeding the day after this accident last Aug its been almost an year. My nurse says my for head oh were all different and I know for fact my for head was the same on both sides not one side higher and you can still see a fist size dark area. I m concerned why the head pain and why still a bruise.

I can understand your concern. The headache, bruise and swelling usually subside in 10-14 days. However sometimes post traumatic headache may persist for years. It usually presents as dull aching ache with episodes if severe pain, pain at injury site, related to stress/exertion, not associated with vomiting, may cause aggressiveness/poor memory/confusion/restlessness.
Even any type of headache can radiate to injury site because of scarring and hypersensitivity in that area.
Post-traumatic headache usually occurs after severe head injury or injury with fracture.

If your symptoms are matching with it, it could be the cause.

Regarding bruising, it might be hyper-pigmentation at injury site. It is usually "brownish" colour and persist for years. It gradually fades away. You can check it noticing its colour. If it flares up intermittently, it could be due to some collection which is getting infected.
I am suspecting it as CT head was normal.

But if it is red/blue, better consult doctor. He will examine you and prescribe CT head to know about injury site, any collection below it.
If there will be any collection, it can be treated by

: Observation only if small in size/no infection/you are getting relief with treatment.
: Surgical evacuation if infected/no decrease in size/large in size/underlying bone eroded.

As you have already mentioned, you are not feeling any abnormality in size of forehead, it is less likely to be collection.
At home, you can take tablet Diclofenac sodium after proper prescription.
