What Causes Heavy Vaginal Bleeding With Clots Post Child Birth?

I had my 3rd son 3 months ago, and my period since has been extremely heavy. I have at least 2 periods in 1 month, I completely soak 3-4 tampons and my back-up pad within 2 hours. I m still having a lot of blood clots. I have an appointment on February 25th, 2016 to talk to my OBGYN about getting my tubes tied, clipped and burned. My mom says that I might have polyps on my uterus and that I may need to have a partial hysterectomy. I m only 22 and would like to know if that could be the problem and if a partial hysterectomy would be a good choice I guess.

I really appreciate your concern, such profuse uterine bleeding could be due to hormone changes but it need to be investigated for other causes too, thinking about the surgery is not good idea but with medical management it will be okay soon hope this information helps.
