* The difficulty in walking and talking in an elderly with high grade fever is indicative of underlying delirium which is usual for such patient with narrated clinical features.
* It is the underlying condition control via medical measures to
- maintain hydration by IV fluids and keep blood pressure under control
- control of blood sugar
- correction of serum electrolytes
- maintain urine output as per the age and weight criteria
- maintain oxygenation of the tissues with peripheral perfusion
- blood culture and intravenous antibiotics as per the clinical condition as that of gram positive , negative organism or atypical ones ; later on change antibiotic as per the susceptibility reporting.
