What Causes Low Grade Fever And Ear Infection In A Child?

I have an infant that is 18 mos old. He was at his dr on Friday for a routine physical. He had a low grade fever 99.4 and dr. said he had double ear infection and was cutting 6 teeth. He has run a fever since. It is about 101-104 and I have been rotating motrin and Tylenol. It doesn t ever take the fever away. Tonight he has a rash on his buttocks and thighs that appear to be hives. Not knowing if I need to take him to the ER tonight.
