I have gone through your question and understood your concerns. You are having painful lump in left testicle, which is diagnosed as infection of the testis (called orchitis) by your doctor. While you are thinking it might be hernia. Well hernia come in scrotum from abdomen through
inguinal canal. there is simple test, if you can go above the swelling it is not hernia rather it is a testicular swelling. And if it is painful, it suggests there is infection. Hernia is a clinical diagnosis and urine routine examination has no role in its diagnosis. Diagnosis is confirmed by blood complete picture, urine routine examination,
ultrasonography of scrotum. If such a patient into my clinic I treat him by prescribing him tab
ciprofloxacin 500mg 12hourly, cap doxycycline 100mg BD, tab paracetamol 500mg 8 hourly and scrotal support. Since mentioned antibiotics are prescription drugs, you need to see a
general surgeon or an
