What Causes Pain And Swelling In The Scrotum After A Hydrocele Removal?

Sept 5, 2017 I had a hydrocele removed, by the 8th I noticed unusual pain and hardness. By phone the clinician--not the urologist said it was normal. I awoke on the 12th at 3am w/ my privates covered in blood because 3 sutures had busted and were bleeding, which I assume was due to my scrotum having swelled to tennis ball size. I got the bleeding stopped with gauze and butterfly band-aids. I called their 24hr service, and even though I told them I felt it was urgent, they said the urologist couldnt see me until the 14th. Note: I know 2 registered nurses. Both agreed, their response and actions didn t seem normal, and so they advised me on what to say and also to record my next appt. on the 14th. By then I was in almost constant pain w/a level 3 out of 10. After requesting twice for a sonogram, the urologist said it was too early to tell, and to come back in 6-8wks. Over the next weeks the urologist wouldnt see me before his 6-8wk time-frame, even after 2 trips to the ER, 2 sonograms, 1 ct scan, each indicating gross abnormalities. But because there was blood flow to the left testicle he said it wasnt an emergency. Fast forward to Nov 10th. My scrotum was now 7.5in long and 20in in diameter I finally found another urologist, who did emergency surgery just to relieve the unbearable pain. He said it was so inflamed inside and scar tissue had engulfed my left testicle to the point he couldn t tell if it was saveable. Afterward, he sent me for more images, but said I ll probably lose it. Plus, he s worried why I m urinating blood/clots. I guess my question is, if I lose my left testicle, 1. will it be due to the prolong effect caused by the 1st urologist refusing to see me. If yes is the answer to 1, then is the 1st urologist liable?

General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Swelling normally occurs after hydrocelectomy and may last for months. However it may be reduced through the following means:
Applying ice packs intermittently over the surgical site for 24 hours.
Wearing of scrotal support for a few days after hydrocelectomy.
Jockey-type underwear, rather than boxer shorts are more helpful in holding the dressings in place.
Taking non-steroidal anti-inflammatory drugs to reduce pain, inflammation and swelling.
Bathing is allowed but dressings must be removed and changed after each shower. The strips of adhesive tape over the incision may be left in place, but the area must be pat dry after bathing.
Constipation must be avoided to prevent straining which increases abdominal pressure and swelling. Foods that are rich in fiber and adequate hydration are recommended. Although one may usually return to normal activities such as driving, walking and climbing stairs, it is better to avoid strenuous activities until swelling subsides. For children, rough play must be avoided.

Hope I have answered your query.
