What Causes Pain At The Back While Urinating?

I am getting pain in my right side at the back when I urinate it feels like something is pulling . I am 45 was in great condition till 3 years ago a high protein diet gave me gallstones and I gained 30 .kgs then. Had a car accident
And was on oxycontin 3 years. tried everythimg to lose weight but could not. Now have gone alkaline lost a few kgs burrow this pulling started.
