What Causes Pain In Breast Radiating To The Arms?

I am 51 5'2" and had a hystercotmy at the age of 28 due to endemetrious. Approx 15 yrs ago I started having a dark green, light green and milky discharge from my left breast. My breast gets to hurting so badly that I have to extract it. My Dr. had me go through the new procedure called ductal lavage, I was the lucky 3rd person in my area to go through the procedure. Needless to say it was painful but all of the tests came out negative. I have yearly mammagrams and every year I have to have it done twice because I have so many fibroids in my left breast. Last year my mother was diagnosised with breast cancer at the age of 76. The pain in my breast has come and gone however in the last 3 months it is almost unbearable to lift my arm above my head due to the pain that radiates. I have noticed that I get a really bad headache and then my breast just throbs until I secreat it. My dr. told me the discharge is from nicotine since I smoke. I have cut down from 2 packs to 1/2 pack a day. This is really annoying and extremely painful what could it actually be?? Nicotine???
