Hello!

Welcome and thank you for asking on HCM!

I understand your concern.

His symptoms and the fact that he is having some effect from omnacortil, indicate an inflammatory disorder of the thoraco-lumbar spine.

You should know that paralysis in the lower half of the body is related to this region of the spine, which is part of the central nervous system.

Many disorders (including inflammatory, demylinization, vascular, toxic, tumoral, etc) could be related to this clinical scenario.

But the fact that he is doing well on omnacortil is an argument in favor of inflammatory or demyelinization disorders.

A brain MRI is necessary to exclude possible multiple sclerosis (which would result in lesions in the brain too).

Further tests (spinal tap, immunological specific tests, infectious diseases tests) are needed to establish the diagnosis.

Anyway, in some cases, the etiology of this disorder, can not be identified even after a complete battery of tests.

Hope to have been helpful!

Best wishes,

Dr. Iliri
