What Causes Penis Swelling With Tenderness And Unusual Discharges?

My 6 year old son complained that his penis was tender. On observation I noticed it was slightly swollen and red and when he retracted his foreskin slightly it has whitish/yellow discharge noted at the top. He has no problems voiding and there is no odor to his urine. Ideas??
