Hi,

It is difficult to provide a specific diagnosis without the results of the MRI and blood work, but some possible causes of persistent diarrhea, vomiting, and fatigue in an 11-year-old child could include:
* Infection: A viral or bacterial infection could cause these symptoms. If the child has recently traveled to an area with poor sanitation, food or waterborne illnesses may be a possibility.
* Inflammatory bowel disease (IBD): Crohn's disease or ulcerative colitis are two types of IBD that can cause chronic diarrhea, abdominal pain, and fatigue.
* Food allergies or sensitivities: Some children may develop diarrhea, vomiting, and fatigue due to an intolerance or allergy to certain foods.
* Autoimmune disorders: Certain autoimmune disorders can cause chronic diarrhea, vomiting, and fatigue.
* Parasitic infections: Parasites such as giardia can cause diarrhea, abdominal pain, and fatigue.

It is good that the child is seeing a rheumatologist, as some autoimmune disorders can cause gastrointestinal symptoms. It is important to continue working with medical professionals to find the underlying cause of these symptoms and develop a treatment plan.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Anesthesiologist Dr. Pallavi.m's  Response
Hello,

The symptoms go in favor of the following conditions:
1) Irritable bowel syndrome
2) Viral gastroenteritis
3) Celiac sprue
4) Chronic mesenteric ischemia

You can once discuss the above possible diagnoses with your gastroenterologist. Also meeting a rheumatologist is a good idea as some autoimmune diseases may present with the above symptoms.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Pallavi M., Anesthesiologist
