Neurologist 's  Response
Hello,

The dosage can be further increased to 4 tablets. However I would suggest to be checked with EKG regarding heart rate. Metoprolol often may not be able to convert it to regular, it is given mostly to keep the rate under control, under 100/min, but it main remain irregular. It is hard to be more precise without more info on type of arrhythmia and test results. Sometimes if an acute onset arrhythmia conversion through drugs or electrical shock may be necessary.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
