1. As there is a history of having hemorrhoids thus my approach will be first to exclude the most common aggravating conditions of rectal bleeding like constipation, polyps, infections and inflammatory bowel disease and as such referral to a General surgeon is necessary.

2. since "physical examination" of the patient is of utmost importance thus I at most can tell you the symptomwise approach that has to be followed

i. examination of stool
ii. Vitals [pulse, heart rate, orthostatic blood pressure]
iii. Inspection of the skin [for pallor] and palpation of the abdomen [for any mass]
iv. rectal examination for fissures, polyps, hemorrhoids.
v. Others: CBC, C Reactive protein, ESR
vi. colonoscopy with biopsy [if needed]

PS. Treatment will depend on the exact cause of rectal bleeding, till then

a. DIETARY REGIMEN

i. take fresh fruits and vegetables, elephant yam, papaya, buttermilk, pistachio, almonds, pear, apple, old rice, crystallized sugar, raw radish and avoid milk and milk products in the diet, an excess of tea, coffee, tobacco, alcohol and carbonated beverages and sitting for long hours.
ii. eat slowly, avoid haste-irregular eating and sweet/heavy food which are difficult to digest and cause indigestion.
iii. take less of strong spices (ginger, garlic), oily-fried-fatty food and flatus producing foods (peas, beans, dals)
iv. take old rice, bottle gourd, moong dal, pomegranate, jaiphal, hot water, adequate rest, and sleep.

v. take fried cumin seeds 500mg,
fried asafetida 125mg,
shweta parpati 500mg, mix them and take with honey three times a day.

- Trifla because of its high nutritional-detoxification and cleansing value is very helpful in treating your present complaints. [take 3 gm with lukewarm water at bed time]

PS:
. Do not strain while passing stool, and use warm water or wet toilet paper but without perfume or alcohol(without soap) to clean your anus after each motion
. Avoid cold/tap/jet (western seat) water.
. Hot sitz bath in a tub of warm water for 10 minutes (2 times a day), use Indian latrine (squatting) instead of western (commode)

General & Family Physician Dr. Dr jagadish golabavi's  Response
Rectal bleeding can be caused by:

hemorrhoids
anal fissure
anal abscess or fistula
diverticulosis/diverticulitis
inflammatory bowel disease (IBD)
ulcers
large polyps
colon cancer

Although rectal bleeding is common, only about one-third of those affected seek treatment. Symptoms usually develop quickly, and most causes are treatable and not serious. In some cases, rectal bleeding can be a symptom of a serious disease, such as colorectal cancer. Therefore, all rectal bleeding should be reported to the primary care physician. He or she can do a rectal examination or order tests like a colonoscopy to determine the cause of the bleeding.
