The reasons of such a severe abdominal pain to keep you in fetal position can be: Ureteric colic due to a stone, appendicitis, volvulus, Ischemic colitis to name a few. You should consult a Surgical Gastroenterologist and get contrast enhanced CT scan of the Abdomen to find out the cause. IT is possible that you will require a surgery.
