General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

All of your symptoms are suggestive of chronic obstructive pulmonary disease (COPD). Are you a chronic smoker? Smoking increases chances of getting COPD.
For proper diagnosis and evaluation consult pulmonologist and get respiratory function tests done to confirm diagnosis of COPD

I hope I have answered your query. Let me know if you have any further questions.
Regards,
Dr. Hiren Hirpara
