What Causes Severe Headaches Along With Nausea?

I have a history of migraines (well controlled), high blood pressure, high cholesterol, high triglycerides, etc. Over the past several days, I have had stabbing/throbbing type headaches, they sometimes cause nausea, and make me feel really sick. I have also had what an ENT thought was BPPV. The supposed BPPV started back in April or May and although I thought it was getting better, over the past couple of weeks, it seems like it has gotten worse again. Do I need to have some diagnostic tests done? I turn 50 in a couple weeks and this is very concerning to me.

Some advices that I can give to you are:     
-Use pain relievers. Aspirin or ibuprofen (Advil, Motrin IB, others) may help relieve mild migraines.
-Use triptans. These medications are often used in treating migraines.
-Use anti-nausea medications if necessary
-Try opioid medications if it doesnt relieve
