What Causes Severe Headaches And Pain In The Right Ear?

Hi I suffer from tremendous migraines and I have for about 8+ years. It’s pretty much daily and one neurologist had me very over medicated but this new one has gotten me down to just a few...BUT I had a major fall backwards off of a front porch in July I was knocked out for a while before anyone realized I hadn’t made it into the cabin, girls weekend in Fredericksburg! Anyway since then I still can’t move my head or neck to the right, I have excruciating headaches stemming from the back of my skull radiating upwards to the rest of my head and very sharp pains straight to my right ear. My CT scan showed empty sella which can be asymptomatic, but can be seen in the setting of idiopathic intracranial hypertension as well as pituitary dysfunction and rounded centrally T1/T2 hypointense lesion within the right maxillary sinus likely inspissated protein versus fungal elements within a mucous retention cyst or polyp. Mild bilateral ethmoidal mucosal thickening. Right temporomandibular joint osteoarthritis. It’s taking me a while to get back to my neurologist Do I need to worry?

General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Since you have so many problems, migraine, empty sella, head injury due to fall, severe pain on right side, neck, ear and so on all the problems need individual consideration one by one so that a combined appropriate management can be done.
First of all consult an Orthopedic surgeon or a Neurosurgeon for the pain in neck, headache and so on for clinical evaluation, examination and proper investigations to have proper management.
Second opinion of an ENT Surgeon helps the best for ENT related problems so that an appropriate medical and/or surgical management can be done.
TMJA Arthritis is in itself a specialized problem and needs the Dental Surgeon working with this to know the exact problem and probable treatment. Till then have liquid diet.
Continue medications properly.
Avoid fire, water, stairs to avoid any accidents.

Hope I have answered your query.
Let me know if I can assist you further.
