What Causes Skin Rash During Winter?

I am a 39 year old male. Over the years I have developed a skin rash that seems to be more prevalent in the winter months. My rash is around my ankles on my calves and the front of my upper legs...and most recently i have been getting it on my hands...little itchy bumps that seem to spread if you keep itching them...i find i itch them the most in my sleep. Any help would be great!! I have used every over the counter cream I could find but nothing seems to help.
