What Causes Sleeplessness With Headaches And Throbbing In The Ears?

I lay down to sleep but only sleep an hour. I feel pounding in my ears, I can t breathe properly and i get headaches. I have to sit up to relieve the systems. I ve had a brain scan, a sinus scan and an echocardiogram revealing nothing? I am a 70 year old male with diabetes and have had a quadruple bypass in 2000.
