Internal Medicine Specialist 's  Response
Hello,

Sharp electric shock-like sensation in the back of the head, especially on lying when the neck is flexed goes more in favour of upper cervical nerve root compression.

Alternatively, it is Lhermitte’s sign, - unpleasant tingling or electric shock-like sensation following neck flexion (active or passive).

It is associated with pathology within the cervical spinal cord (Multiple sclerosis). MRI cervical spine will clarify.

Hope I have answered your query. Let me know if I can assist you further.

Take care
