What Causes Upper Abdominal Pain In A Diabetic Patient?

Hi Dr. My husband is 57 yrs old with high blood sugar problems been on medication for about 4yrs an still is high Drs.want to put him on insulin could upper stomach pain be due to pancreas not working properly any more he has been having upper stomach pain starting in the middle and going to the right upper stomach area the pain has been keeping him up most of the night tried tums and liquid antacids but doesn t seem to help at all he says at the end of the day he feels like all his food is staying in upper stomach area he is very thirsty and drinks a lot of water but can t seem to consume even liquid at end of day says he feels very full.

Hi,

Epigastric pain, that is the upper stomach pain is due to gastritis. Gastritis is the condition of the stomach in which the lining of the stomach gets inflamed. Since, your husband is on diabetic drugs for long time, it indicates that the cause for gastritis is Oral hypoglycemic agents which are prescribed to control blood sugar level. Apart from drug induced gastritis, irregular diet, spicy and oily diet also lead to gastritis due to excess gastric acid secretion. Mucous layer is the one that protects the stomach from gastric exposure. Excess acid and fasting leads to mucosal erosion and exposure of the epithelial lining which leads to injury and inflammation.

Endoscopy is the investigation of choice for gastritis, duodenitis and GERD. Abdominal ultrasound helps to rule out any other pathology. Proton pump inhibitors or H2 receptor blockers are the drugs that are used to decrease the gastric acid secretion thereby decreasing the chances of mucosal injury. Antacids are also prescribed to neutralize the excess acid after digestive process. Sucralfate and digestive enzymes are prescribed to coat the damaged mucosa and to digest the dietary products respectively. Visit the gastroenterologist for further evaluation and management.
