The pepper like lesion on the anus and perinium is cause due to hair end infection called as Folliculitis... to get cured, personal hygiene is must., local application of Erythromycin cream may be useful... Do not prick with the finger, it will add further infection to the follicle... you can take opinion of Dermatologist..
