Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
What Do Apical Pleural-parenchymal Changes In Lungs Signify?
I had a simple chest x-ray done as part of medical clearance for spinal surgery. X-ray showed apical pleural-parenchymal changes and presence of cardiophrenic fat pads. I am a 46 year old female with a less than average weight. I have had frequent episodes of bronchitis and pneumonia throughout my life and have been diagnosed with reactive airway disorder. However, this is the first time the above findings have been noted on my chest x-ray. Is this something I should have checked out prior to surgery?
Thanks for asking at Healthcare Magic. I read your question and I understand your concern. Apical pleural parenchymal changes implies the following 1. Extrinsic allergic alveolitis 2. Pulmonary tuberculosis 3. Sarcoidosis 4. Allergic broncopulmonary aspergillosis 5. Benign lesion While cardiphrenic fat pad is due to either Coronary artery disease, Viseral obesity, pericardial effusion, exogenous steroid administration, morgagni hernia, pericardial cyst Any how cardiophrenic fat pad is incidental finding and doesn't need treatment. I would like to advise you to have CT thorax with contrast and Echo, ESR to settle the diagnosis. However before surgical intervention do your PFTs if FEV1 was less than 200 than it is contraindication for elective surgery. Hope it helps best.
