What Do The Following Biopsy Result Indicate?

In September my psa was 4.22 down from a year ago at 4.98, I had a psa test last week my psa is at 5.17 and my free psa is at 13% down from 16% and14% in my last 2 tests. I had sex less than 12 hours before and cycled on my stationary bike for 45 minutes. I had 2 biopsies 4 years ago and 2 years ago, both negative but with A Typical and neoplasia. Just wondering should I have another biopsy.
