What Does Gland Parenchyma Of Transitional Zone Indicate?

I have BPH wt 53gm,PSA9.17 with acute prostatitis.urine culture was positve proteus pus cells>10^5cfu sensitve to 17iteams.1 mo.back taken nitrofuradantn responded well but now I have UTI.

PSA on 5/11 is 4.36,free PSA1.07&trnsrectal usg shows Glandparenchyma of transitonal zone displays a hetrogeneous echogenicity with focal cystic lesion in Rt lobe.5.2x4.8mmhypo echoecoic lesion.my age is 62yrs wt 65 kg.pl advice your line of tratment & chances of cancer? Thanks.Dr.P.M.Srivastava

Hello and welcome to HCM,

The previous episode of acute prostatitis and urinary tract infection has been treated by antibiotic.
Currently the PSA level has fallen after treatment of acute prostatis.
The ultrasonographic findings suggest presence of beningn prostatic hyperplasia (BPH).
In BPH, cystic changes are common.
In case of prostatic cancer, the level of PSA is elevated.
Another parameter- ratio of free PSA to total PSA is also elevated.
If the ratio is less than 6%, there are chances of cancer of prostate and if the level is more than 23%, BPH is more likely.
In your case the ratio is 24% thus, likelyhood of cancer is low.
You can get PSA and free PSA levels on yearly basis to look for increase.
