Your husband's semen analysis report suggests decreased sperm motility with excessive abnormal morphology of sperms. At least 55 % sperms should be motile. At least 16 % sperms should have normal morphology.
Your husband should do exercise and take nutritious food. He should avoid tight underpants. Avoid cigarettes and alcohol if he take these things. If low sperm motility and abnormal morphology persists then you may need assisted fertilization techniques like IUI.
Other factors like sperm count is within normal limit.
