Hi,
T1 and T2 weighted images are the sequences of MRI done in order to differentiate between grey and white matter of brain.Usually there will be demarcation seen between white matter and grey matter of brain parenchyma.Abnormal diffuse appearance can be early feature of cerebral edema. If you are not symtomatic (headache, vomiting, visual obscuration)you need not worry. This abnormal diffuse appearance can also be artefacts of MRI.Prominent perivascular spaces are spaces around blood vessel which is also normal in your case.

Internal Medicine Specialist Dr. Kiran Kumar's  Response
Hi. Thanks for your query.

Well I could have given you a more specific answer if you had given your clinical details of your problems.

Your report says, there are abnormal diffuse appearance of brain parenchyma in T1 and T2 weighted images and Incidental finding of a prominent perivascilar ? (Either periventricular or perivascular) spaces.

Now if the final report says, Normal MRI of the brain, we would go by that for your age.

Incidental finding means, that it was not expected and it does not carry any significance if overall report is normal.

So, be reassured. If you are still concerned, mail me the MRI images with your clincal details; I shall help you further.

Thanks
