Hi, My wife s MRI came back with the following results. what does this mean? any help will be appreciated. Again seen are multiple T2/hyperintense lesions in the subcortical white matter       bilaterally. These are grossly stable in number and distribution, with no       significant interval change allowing for slight differences in technique. No       definite cerebellar or brainstem lesions are seen. Postcontrast images       demonstrate no significant enhancement associated with these lesions, or       elsewhere in the brain parenchyma. There is no mass effect or midline shift.       No abnormal extra-axial fluid collections.       Mucoperiosteal signal is thickened in the left maxillary sinus and to a lesser       degree in the right maxillary sinus, sphenoid sinus, ethmoid air cells, and left       frontal sinus. No fluid levels are seen. No mastoid fluid. Thank you. Diwakar

Thanks for writing to HCM

Multiple T2/hyperintense lesions in the subcortical white matter bilaterally may be due to chronic microvascular ischemic changes resulting from involvement of small blood vessels,demyelination,post traumatic sequel etc.
Since it is not progressed,ischemic changes are likely.
Chronic microvascular ischemic changes are due to ischemic changes resulting from blood vessels changes in diseases like hypertension,diabetes mellitus,dyslipidemia.
So,she need monitoring of conditions that leads to ischemic changes like hypertension,altered lipid profile,diabetes mellitus.
She need investigations like routine hemogram,RBS,LFT,RFT,Lipid profile,ultrasound of abdomen.
She is also suffering from left maxillary sinusitis and to a lesser degree in the right maxillary sinus,sphenoid sinus,ethmoid air cells and left frontal sinus.
