What Does T4 Of 13.8 And TSH Of 6.41 From Blood Test Suggest?

DOB: 10. August. 1987
She has had bloods done which show a free T4 of 13.8. Her TSH level was 6.41. I have advised her to have this re-checked. She also has an LH of 13.8 with an FSH of 64. Prolactin level was 116 and her serum progesterone was 3. This was in December. In August it was 2.6. I have been advising her to keep records of her periods, but they appear to be occurring roughly every 8 to 10 weeks. A scan of her ovaries was done in August 2011. The patient only allowed them to do a transabdominal scan which was reported as showing sub-optimal views of the ovaries. However there were small peripheral follicles seen.

Hi,

1. Regarding thyroid - she is having mild under functioning of thyroid - her TSH is high. She should be started on thyroid medications after consulting a physician.

2. Regarding ovarian function - Her FSH is very high - is it taken within 1 week of menstruation? If not high FSH can be normally seen in a women at the time of ovulation. Prolactin is also high.

This could all be a result of polycystic ovarian syndrome (PCOS). I suggest you to do blood test (LH, FSH,prolactin) within the first week of next menstruation. If it is suggestive of PCOS there are treatment to regularise your cycles.
