Hello and welcome to HCM, A pleura based nodular density in lower lobe of lung needs to be investigated. The pleura based lesion suggests that it is arising from the pleura and it may be benign or malignant. A history of smoking for 25 years and a family history of lung cancer increases the possibility of malignant tumor but relevant investigations need to be done. Benign tumors of pleura include solitary fibrous tumor, benign mesothelioma etc. Malignant tumors include malignant mesothelioma, metastasis from surrounding structures. Further investigations like pleura biopsy will be required to diagnose the lesion. Benign tumors can be treated by simple surgical excision. Malignant tumors require more aggressive treatment - surgery and chemo / radiotherapy.
Thanks and take care Dr Shailja Puri
